Healthcare call center for IPS
If your appointment center runs on busy lines, schedules on a separate screen and reports in Excel, here is the full process and what you should require.
This guide is for you if you run or coordinate the appointment center of an IPS (instituciones prestadoras de salud, Colombia's healthcare provider institutions), or if you are part of the administrative management accountable for it. It explains how a healthcare call center works in Colombia, where appointments get lost and what to check before you contract a platform, with a checklist you can use to evaluate any vendor.
In short
- The actual work: the center answers, schedules, confirms, reschedules, guides patients on authorizations and receives PQRS (petitions, complaints, claims and suggestions) across several channels, and every contact leaves data the regulations require.
- Where appointments get lost: saturated lines, schedules on another screen, reminders done by hand and freed slots that nobody reassigns.
- No-shows matter: a systematic review of 105 studies from several continents estimates an average no-show rate of about 23%.
- What to require: scheduling inside the conversation, the data required by Resolución 1552 de 2013 (Resolution 1552 of 2013) on every appointment, the same metrics for people and AI, and campaigns with contact hour controls.
- The regulations: Resolution 1552 of 2013, Ley 2300 de 2023 (Law 2300 of 2023), Ley 1581 de 2012 (Law 1581 of 2012) and Ley 1755 de 2015 (Law 1755 of 2015) affect the center's daily work.
How an IPS appointment center works
Almost every center starts with one line per site, each with its own team and its own way of taking notes. Over time the IPS consolidates everything into one number, sometimes a toll-free 01 8000 line, and puts an IVR in front of it: the menu that asks for the site or service, announces opening hours, gives notice of holidays and says whether demand is high.
From the IVR the call moves to a queue, organized by service or by skill (general medicine, specialists, dentistry, authorizations), which distributes the work according to who is available. Priority populations, such as pregnant women, older adults or people with disabilities, should move through faster.
When someone on the team answers, the cycle is almost always this:
- Identify the patient by ID document and confirm they are who they say they are.
- Listen to the reason: a new appointment, rescheduling, cancellation, authorization or complaint.
- Look for open slots in the schedule of the clinical software (HIS, the hospital information system) and assign one.
- Record the appointment with the date it was requested and the date the patient wanted.
- Close with a disposition, the code that says what happened in the contact.
Other tasks surround this cycle: confirmation calls, guidance on authorizations from the EPS (the health insurers that manage Colombia's mandatory health plans) and PQRS, which have a filing number and legal deadlines.
The operation is measured with these indicators:
- Service level: the percentage of contacts answered within a threshold of seconds that you define.
- Abandonment: contacts that hang up or leave while waiting in the queue.
- ASA: average wait time before someone answers.
- AHT: average handling time, talk time plus wrap-up.
- FCR: cases resolved on the first contact, without the person calling back about the same issue.
- Timeliness: the wait between the appointment request and the assigned date, counted in days.
- No-shows: assigned appointments the patient did not attend.
The problems repeat as well. At peak hours the lines saturate and abandonment rises. The schedule lives on another screen and the agent copies data between two systems. Reminders are done by hand, when there is time. A slot freed at the last minute never reaches anyone on the waitlist. Each site's WhatsApp sits on a phone with no record. And the timeliness report is put together in Excel at the end of the month.
Timeliness matters more than it seems: the same review found that a long wait until the appointment and previous no-shows are the most frequently reported factors behind no-shows. To go further, see how to reduce no-shows for medical appointments and how WhatsApp appointment scheduling works.
What a call center for an IPS should have
Use this list to compare vendors, Omcall included. If a platform lacks something that is mandatory for your IPS, rule it out even if it costs less.
- Scheduling inside the conversation: the agent books without switching screens, against your real HIS. Ask how it connects and what happens when the HIS does not respond.
- Resolution 1552 data on every appointment: request date, desired date, assigned date and the site's REPS code (Registro Especial de Prestadores de Servicios de Salud, Colombia's registry of health service providers), saved at that moment and not editable afterwards.
- All channels in one queue: voice, WhatsApp, web chat and email, with a single history per patient.
- An IVR your team can change: hours, Colombian holidays, contingency messages and callback.
- Rule-based priority: so that priority populations do not depend on the agent's memory.
- Confirmation and waitlist: automatic reminders and reassignment of freed slots.
- Identity verification before sharing health data.
- Campaigns with contact rules: Law 2300 hours, exclusions and a record of every block.
- Comparable indicators: the same formula for service level, AHT and FCR for people and for AI.
- Live supervision: listen in, whisper and barge in on a call.
- Protected data: notice before recording, encrypted recordings, configurable retention and a log of who listened to each recording.
- Clear costs: what the license covers and how minutes, messages and AI are billed.
- Your numbers and your carrier: use your current lines or your own SIP carrier.
How Omcall handles it
Omcall brings voice, WhatsApp, web chat, email, Messenger and Instagram into a single inbox. Agents work in the browser with a softphone and handle one call and several chats at the same time. A single ACD distributes work to people and to AI agents, with skills, last-agent affinity, overflow between queues, callback and priority for priority populations according to your IPS's rules. You can use your own numbers and your SIP carrier.
The IVR and the bots are built in a visual editor with versioning, a simulator, Colombian holidays and contingency messages. Scheduling happens inside the conversation: Omcall integrates natively with Omneri and connects to other clinical software that exposes FHIR R4; for a system without FHIR, a connection to its API is assessed during implementation.
Every appointment, whatever channel it comes from, stores the Resolution 1552 data (request date, desired date, assigned date and site) with no way to modify it, and timeliness is measured against 3 business days, with holidays taken into account. For specialists, the search covers the full year before offering the waitlist. Reminders go out at 72 and 24 hours over WhatsApp with buttons to confirm, reschedule or cancel, and a canceled slot is offered to the waitlist in rounds: the first person to accept gets it.
Before showing health data, the agent verifies identity with the ID document plus an additional detail, or with a one-time code. Orders that arrive over WhatsApp are read with AI vision and go to human review when the reading is not reliable. PQRS get a sequential filing number, Law 1755 deadlines counted with holidays, and alerts before they expire. Calls play the data processing notice before recording, and every access to a recording is audited.
Supervisors see people and AI on a live dashboard, listen in, whisper, barge in and can take over an AI conversation. Reports use the same formula for service level, abandonment, ASA, AHT, FCR and occupancy for everyone, and include timeliness and no-shows by specialty and site, with Excel export and an API for BI. Quality transcribes each speaker separately and evaluates 100% of interactions.
The guide on AI agents for call centers explains the ten agents and their limits. Everything else is on the platform page and in a demo with your own cases.
Applicable regulations
- Resolution 1552 of 2013: requires EPS, directly or through their provider network, to keep specialist schedules open every business day of the year, not to exceed 3 business days for general medicine or dentistry unless the patient asks for another date, and to record the request date, desired date, assigned date and the IPS's REPS code. The text is available in the regulatory compendium of the Superintendencia Nacional de Salud (Supersalud, Colombia's National Health Superintendency).
- Law 2300 of 2023: sets hours and frequency for debt collection and extends those hours to commercial messages (official text). It affects payment campaigns and any promotional message.
- Law 1581 of 2012: health data is sensitive and, as a general rule, processing it requires explicit authorization. Habeas data inquiries must be answered within 10 business days and claims within 15. This is how Omcall handles data: privacy policy.
- Law 1755 of 2015: governs the right of petition; the general rule is a response within 15 days, and within 10 for requests for documents and information.
Omcall implements these rules as configurable parameters. The deadlines, windows and texts it ships with by default are drafts to review with your legal advisor: no platform certifies compliance for you.
Frequently asked questions
It is a contact center designed around the cycle of a medical appointment, not around sales. Besides answering calls, it has to check the schedule in the clinical software, verify identity before discussing health data, record the dates the regulations require and measure timeliness. A generic commercial call center usually does not include those pieces. In Colombia, Resolution 1552 of 2013 requires recording, for every appointment, the request date, the desired date, the assigned date and the IPS's REPS code.
It should measure the indicators of any contact center plus the healthcare ones. The first group is service level, abandonment, ASA (average wait), AHT (handling time) and FCR (first contact resolution). The second is timeliness in assigning appointments, the no-show rate and the confirmation rate. If you use AI agents, measure them with the same formula as people so you can compare them. For reference, a review of 105 international studies published in Health Policy in 2018 estimated an average no-show rate of about 23%.
Up to 3 business days from the request, unless the patient asks for a later date. This is set by paragraph 3 of article 1 of Resolution 1552 of 2013, which applies to general medicine and general dentistry. The obligation falls on the EPS, but it is met through their provider network, so the IPS appointment center is where the data is generated. There is no fixed deadline for specialists, but their schedule must be open every business day of the year.
Yes, as long as the conversation is connected to the schedule and leaves the same record as a phone call. The risk lies in the WhatsApp on a site's phone: it leaves no audit trail, does not measure timeliness and nobody supervises it. In Omcall, WhatsApp enters the same queue as voice, the agent or the AI verifies identity before scheduling, and every appointment stores the data Resolution 1552 requires. The guide to WhatsApp scheduling explains the full flow.
No. Omcall integrates natively with Omneri, but it also connects to other clinical software that exposes FHIR R4, the version of the HL7 standard for exchanging health data between systems. If your HIS does not support FHIR, a connection to its API is assessed during implementation. In the demo we review what your HIS offers, what data the scheduling needs and which option makes the most sense in your case.
Not necessarily, and it is best for your legal advisor to decide. Law 2300 of 2023 regulates debt collection and extends its hours to commercial messages: Monday to Friday from 7:00 a.m. to 7:00 p.m., Saturdays from 8:00 a.m. to 3:00 p.m., and never on Sundays or holidays. An appointment reminder is informational, but the interpretation is not settled. That is why Omcall makes the reminder category configurable per IPS and applies the Law 2300 hours and frequency to collection and commercial campaigns.
Omcall is billed as a license per agent per month plus AI and channel usage, such as voice minutes and messages. The price depends on how many people handle contacts, how many sites and channels you connect and which AI agents you turn on, so it is sized in the demo with your real operation, and that produces a specific figure, not a range. IPS that join as design partners get a 30-day pilot at no cost. You can book the demo.
